Before we begin, please fill out this short questionnaire so that I can be well-prepared for our first meeting. 

Zoom is my preferred meeting option, and I will send you a link after we schedule our first session. We can also meet via phone or Skype if you prefer. You will not need to use your camera. These are voice only calls. All sessions will be recorded, if you wish, for your reference later. 

When you are ready for your first coaching session, please contact me at my customer service desk (https://swcfavs.com/support) and request an appointment, along with the best times that work for you. I'll respond back and we'll setup a time for our first session.
